This is the new Annual Health and Medical Record form. It is
replacing the current Class 1, 2 and 3 health forms.
The major differences are -
Everything is now in one place.
It is a fillable, and savable, pdf form.
For activities lasting over 72 hours a medical evaluation by a
licensed health-care provider is now mandatory.
In the medical evaluation section (B) there is a height/weight chart
that MUST be strictly adhered to if the event will take the unit
beyond a radius wherein emergency evacuation is more than 30 minutes
by ground transportation.
This form is for both youth and adult.

Too bad that weight requirement will knock people out of activities that don’t let anything stop them from providing quality programs. Mumble grumble.

How many of you are aware of “nature deprivation syndrome?” This is caused by boys never having any experience in nature and parents that hover/overprotect. Rules that limit leadership participation limit the program.
